Dental Insurance in Lakewood, CO
We take most PPO insurances, and easily submit applicable claims to your insurance provider. We work hard to gather all relevant information pertaining to your insurance plan that will best determine the cost of your treatment. However, these are usually estimates and not necessarily a guarantee of payment or coverage. Any services that are not covered entirely by insurance will be the patient’s responsibility.
Please take note that we do not accept HMO/DMO insurance plans or Medicaid at this time.
Belmar Smiles Insurance Plan
Our affordable in-office insurance plan is available to all patients who do not have insurance coverage. The plan includes two annual dental exams, two annual routine cleanings, X-rays, oral cancer screening, and fluoride treatments. Members also receive a 15% discount on treatment services. The cost of the plan is $450 annually for adults and $350 annually for children under the age of 14.
Other Payment Options for Dental Patients
For services not paid by insurance, we accept cash, checks, and most credit cards as payment for dental treatment. We also offer convenient online payment access by using the attached form.
